Does your care center have art images and stories to share for our newsletter, website and trainings?  We love showcasing the great work you're doing!  Our newsletters are shared with the Centers for Medicare & Medicaid Services and other federal and state agencies - in addition to our many newsletter subscribers.  

Art images and the stories behind them must be submitted via this tool due to the volume Thriving4Life receives.  Please fill in the blanks and use the links.  Don't forget to provide a Media Release form for each artist and staff person that shares!  We cannot publish without a Media Release.  If you have previously submitted a release for an individual, you are not required to submit again.  We keep all releases on file.  Thank you!!!
